Features of antithrombotic treatment in exacerbation of coronary disease and percutaneous coronary interventions in patients with atrial fibrillation, who take vitamin K antagonists (literature review)

The review analyses approaches to management of patients with atrial fibrillation, who have to take vitamin K antagonists in order to prevent arterial thromboembolic events in case the patient has an exacerbation of coronary disease and/ or requires a percutaneous coronary intervention. Because the problem is little-studied, the majority of opinions about the most effective and safest modes of administering antithrombotic agents in the above-mentioned clinical situations, are based on the results of follow-up of the patients, the information about features of the action of drugs, and common sense. Current recommendations are presented for long-term antithrombotic therapy in patients with atrial fibrillation, who take vitamin K antagonists and have undergone coronary stenting.

Transluminal balloon angioplasty in the treatment of lower limb critical ischemia

Purpose of the study. To evaluate the efficacy of transluminal balloon angioplasty, or percutaneous transluminal angioplasty (PTA) in the treatment of critical lower limb ischemia (CLI). Materials and methods. Between November 2004 and April 2008, 89 PTA were performed in 82 patients with CLI. The patients’ age ranged from 45 to 89 years; mean age 67.8+9.8 years. 50 (61%) patients had ischemic foot ulcers, 17 (20.7%) had gangrene and 15 (18.3%) suffered from ischemic rest pain. 45 (54.9%) patients had diabetes mellitus, including 33 (40.2%) with insulin-dependent diabetes. There were 60 (72.3%) patients with coronary disease, 65 (79.3%) with arterial hypertension, 25 (30.5%) with cerebrovascular disease and 13 (15.9%) with chronic renal failure. 6 (7.3%) patients were on chronic hemodialysis. Distribution of cases according to the level of lesion was as follows: iliac level – 4 (6.3%), femoropolitelial – 55 (49.5%), lower leg arteries – 52 (46.8%). Type C and type D lesions (TASC) occurred in 89.2 % cases. Occlusions were observed in 82% of all lesions. Subintimal angioplasty was performed in 42.2 % of the patients. Stenting was used in 14 (7.3%) cases. Results. Angiographic and clinical success of the procedure was achieved in 75 (91.5%) patients. No CLI was observed at one and three years’ follow-up in 79.4 and 52.9% of the patients, respectively.

The main topics of the congress touched upon issues related to the application of the techniques of thermal ablation and sclerotherapy for varicose veins, prevention and treatment of venous thrombosis, new research in the field of compression therapy. The plenary meetings started developing evidence-based guidelines for the management of patients with lymphedema and venous malformations. Also some Russian reports presented at the forum are reviewed.
